How hard does Pandemic get? We left the world alone and timed it
Pandemic has one difficulty dial: the number of Epidemic cards in the player deck. Introductory uses 4, Standard 5, Heroic 6. On the Brink adds Legendary with 7. Every Epidemic speeds up the infections and throws 3 cubes on a fresh city.
To see what one more card really does, we played the decks with nobody at the table. No treating, no cures, no events. Just the infection deck, the Epidemics and the outbreaks, following the rulebook to the letter, on the real map.
How long an untreated world lasts
Share of worlds still standing after each turn, 4 players, nobody acting. 200,000 worlds per setting.
- Introductory (4)
- Standard (5)
- Heroic (6)
- Legendary (7)

That gap is the honest measure of difficulty, so we turned it into the "How hard it gets" meter in the ranking below: the share of untreated worlds that are gone by turn 8, out of 10. Each extra Epidemic adds a point or two. Legendary is not a small step up from Heroic.
Out of 10 untreated worlds, how many are gone by turn 8
The 'How hard it gets' meter. 4 players; Legendary uses On the Brink's rule of 2 events per player.

Why double Epidemics (almost) never happen
The setup rule everyone rushes through is the clever part. You split the player deck into as many piles as there are Epidemics, shuffle one Epidemic into each pile, and stack them. So there is exactly one Epidemic per stretch of the deck, and they can't bunch up the way a plain shuffle would allow.
Two Epidemics in one draw can only happen when your 2-card draw crosses the seam between two piles. Whether that is possible at all depends on whether the piles have an even number of cards. At Standard with 2-4 players they always do, and so does Legendary with 4. Hover the orange marks to see where Heroic's seams fall.
The player deck, pile by pile (4 players)
Each block is one pile with its Epidemic. Lines below are the 2-card draws, one per turn; orange draws cross a seam.
Show the numbers
| Setting | Cards per pile (plus 1 Epidemic) | Double Epidemic | Turns in the deck |
|---|---|---|---|
| Standard | 9, 9, 9, 9, 9 | 0.0% | 25 |
| Heroic | 8, 8, 8, 7, 7, 7 | 5.7% | 25 |
| Legendary | 7, 7, 7, 7, 7, 7, 6 | 0.0% | 27 |
| Double Epidemic | Epidemics in back-to-back turns | Back-to-back if you shuffled them anywhere | |
|---|---|---|---|
| Introductory | 2.0% | 10% | 49% |
| Standard | 0.0% | 16% | 69% |
| Heroic | 5.7% | 31% | 83% |
| Legendary | 0.0% | 34% | not computed |
The split also protects you from the worst streaks. Shuffle the 5 Epidemics anywhere and 69% of games would have two of them in the same or the next turn. With the piles it is 16%. Three Epidemics within three turns, which a plain shuffle produces 16% of the time, cannot happen at all.
The deck math has one more lesson, about what an Epidemic does to the cities that were already infected. It is the rule that loses most first games, so we put it in Pandemic vs Pandemic Legacy.
The ranking
We ranked by how much good Pandemic each box gives you: how long it stays fresh, which groups it works for, how far it pushes the difficulty, and whether you can still buy it new (as of November 2026). "Replay value" and "Easy to teach" are our ratings. "How hard it gets" and "Sessions" are computed. Hover any meter to see what feeds it.
1. Pandemic Legacy: Season 1
Best overallLegacy, 2015
The best Pandemic game. You just only get to play it once.
- Starts as plain Pandemic, so new players are fine from game one
- 12 months, each with a second chance if you lose the first try
- Lose a game and the next one gets 2 more funded events; win and it gets 2 fewer

Each game is a month of a story. Between games you open sealed boxes, put stickers on the board and the cards, and tear some cards up. Cities that suffer outbreaks get less stable from game to game, characters pick up good and bad traits, and the rules grow as the story needs them. The red and blue boxes are the same game.
Unsure between this and plain Pandemic? We wrote a whole page on it: Pandemic vs Pandemic Legacy, including how many evenings a campaign really takes.
- Played once: the campaign changes the box for good
- Needs the same group every time
2. Pandemic
The hubBase game, 2008
Still the box to own if your group changes, and the one every expansion needs.
- 45 minutes, 2-4 players, three difficulties in the box
- 48 city cards, 5 events, 7 roles
- Plays the same on night 50 as on night 1, which is the point

Four diseases, a world map and a deck that keeps getting meaner. The box in shops is the 2nd edition (2013), which added two roles. You win by curing all four. You lose on the 8th outbreak, when a color runs out of cubes, or when the player deck runs out. Our meter scores its hardest setting, Heroic: 63% of untreated worlds are gone by turn 8.
- Tops out at 4 players without On the Brink
- One confident player can end up running everyone's turn
3. Pandemic: On the Brink
Best expansionExpansion, 2009
The expansion to buy first: more of everything, and the hardest difficulty we measured.
- 6 new roles and 8 new events
- 3 challenges: Virulent Strain, Mutation (a 5th disease) and Bio-Terrorist (one player turns on the rest)
- Plays 5, and comes with petri dishes for the cubes

First released in 2009 and revised in 2013. Legendary is the headline: a 7th Epidemic card. On the Brink also changes how many events go in the deck, to 2 per player, which makes the deck a little longer. The math says the extra Epidemic wins that fight easily:
- Needs the base game
- Some challenges add a fair bit of bookkeeping
4. Pandemic Legacy: Season 2
Best second campaignLegacy, 2017
Bolder than Season 1 and further from regular Pandemic. Play it second.
- You start with a small known world and discover the rest
- A prologue you can replay to learn the new rules
- You design your own characters

Set 71 years after Season 1, in a world the plague has emptied. Instead of curing diseases you mostly keep cities supplied, travelling by sea between a few safe havens. The yellow and black boxes are the same game.
- More new rules to absorb than Season 1
- Same deal as every Legacy: one campaign, then done
5. Pandemic Legacy: Season 0
Spy themeLegacy, 2020
A Cold War spy campaign on the Pandemic engine. The pick if the outbreak theme puts you off.
- 1962, CIA recruits, a Soviet bioweapon to stop
- Aliases and passports give your characters a real identity
- A prequel: you don't need the other seasons

Season 0 sits between the other two: closer to regular Pandemic than Season 2, further than Season 1. It was the third Legacy season, and as of November 2026 still the last one we can confirm with Z-Man.
- Played once, like the others
- Further from regular Pandemic than Season 1
6. Pandemic: State of Emergency
For veteransExpansion, 2015
Three sharp challenges for groups who have beaten Heroic. Now hard to find.
- Hinterlands: diseases jump from animals into cities
- Emergency Events: nasty surprises shuffled into the player deck
- Superbug: a 5th disease you can't treat, only vaccinate against

Each challenge is a module you add to the base game, so you can pick the flavor of pain. We left it out of the difficulty meter: Emergency Events change the player deck itself, and we only computed decks we could check card for card.
- Z-Man lists it as currently unavailable (November 2026)
- 2-4 players only
7. Pandemic: Iberia
Best new settingStandalone, 2016
Regular Pandemic rules in 1848 Spain and Portugal, with more texture. Our favorite standalone.
- Malaria, typhus, cholera and yellow fever, each with its own quirk
- You build railways to get around
- 2-5 players

Designed by Jesús Torres Castro with Matt Leacock. It began as a limited edition and still turns up as Pandemic: Iberia.
- Availability comes and goes
- If you already love regular Pandemic, the change is mostly in the details
8. Pandemic: Fall of Rome
Plays soloStandalone, 2018
Barbarians instead of diseases, battles instead of treating, and it works from 1 player.
- 1-5 players, including solo
- Make alliances with tribes instead of only fighting them
- Dice battles add a bit of drama
- Z-Man lists it as currently unavailable (November 2026)
- Dice add luck that pure Pandemic fans may not want
9. Pandemic: Hot Zone
Best short gameStandalone, 2020
Pandemic boiled down to its core: three diseases, a small map, thirty minutes.
- 30 minutes
- Small box, quick to teach
- Two boxes, two maps: North America 2020, Europe 2021
- Less to discover than the full game
- Both are listed as currently unavailable by Z-Man (November 2026)
10. Pandemic: Rising Tide
Most originalStandalone, 2017
The Netherlands against the sea: water flows instead of disease cubes.
- Water spreads along the map, so where you build dikes really matters
- A theme nobody else has done: holding back the sea
- 2-5 players
- Z-Man lists it as currently unavailable (November 2026)
- The water rules take a game to click
11. Pandemic: In the Lab
Up to 6Expansion, 2013
Adds a lab where you research the cures. Clever, but it can feel like two games at once.
- Team mode for up to 6, and a solo mode
- Curing becomes a puzzle instead of a card count
- Needs the base game, and Z-Man lists On the Brink as required too
- Your attention splits between the map and the lab
The spin-offs and Pandemic System games
Z-Man now calls the engine the Pandemic System and puts it under other themes, sometimes without the word Pandemic on the box. These are good games, but they are their own thing, so we didn't rank them against the boxes above.
Newest (2025), 1-5pThe Lord of the Rings: Fate of the Fellowship
The Pandemic engine follows the Fellowship. Easy to buy new as of November 2026.
Horror, 2-4pPandemic: Reign of Cthulhu
Investigators race to seal the gates before an Old One wakes. Pandemic with tentacles.
Fantasy, 1-5pWorld of Warcraft: Wrath of the Lich King
Warcraft heroes take on the Lich King's undead on the familiar engine.
Star Wars, 1-5pStar Wars: The Clone Wars
Heroes of the Republic against the Separatists. Same engine, very different paint.
Real time, 2-4pPandemic: Rapid Response
Dice and a sand timer: deliver supplies before it runs out. Chaos in 20 minutes.
Dice, 2-5pPandemic: The Cure
Pandemic as a dice game. Quicker and luckier than the board game.
Competitive, 2-5pPandemic: Contagion
You play the diseases and race each other to wipe out cities. Not cooperative at all.
Which Pandemic should you buy first?

Legacy: Season 1 Pandemic On the Brink
How we ranked, and what we computed
The ranking is our editorial call, weighted toward replay value, player range and how much a box changes the game, and away from novelty. Product facts (players, years, minutes, availability) come from Z-Man Games' product pages, the rulebooks and BoardGameGeek, as of November 2026.
The deck numbers are exact. Each Epidemic sits uniformly at random in its own pile, so we enumerated every possible layout of the player deck with its exact probability. For the "leave it alone" curves we simulated 200,000 worlds per setting on the real map (93 connections between 48 cities), with the outbreak, chain reaction and cube-supply rules checked by tests against the rulebook. Nobody acts in those worlds, so they measure the pressure of each setting, not your chances of winning. Expansions that change the player deck in ways we did not model (State of Emergency, In the Lab, the Legacy seasons) get no "How hard it gets" meter. More on how we work.
